No it doesn't fall over. very sturdy with the handle up

Q: When using as a backpack, do the wheels/base dig into your back/hips?

I think that depends on your height. Straps are adjustable and there is a cover to stop dirty wheels ruining your clothes.

Q: Can you confirm the backpack straps have the the extra support strap across the chest, as pictured? (As it is missing in the customer review photo).

I got the 45x36x20 size of this bag, and there is no extra strap across the chest. However, I don't know about the larger size. Hope that helps

Q: Does the 55cm one is 55 including the wheels and the handle (when the handle is folded)

We bought the 45 cm high one and definitely the height include the wheels and the handle is fully folded inside the back of the bag
